For Public Use, Program Participation, and Visitor Engagement
The First Tee - Virginia Blue Ridge Youth Development and Golf Performance Facility ("The Clubhouse") is a welcoming community space designed to provide youth and families with opportunities to develop golf skills, learn life skills through the game of golf, and become familiar with the mission, programs, events, and participation opportunities offered by First Tee - Virginia Blue Ridge. The Clubhouse may be used for golf practice, player development, family engagement, educational activities, program exploration, and participation in First Tee experiences.
I, the undersigned participant, visitor, prospective participant, or parent/legal guardian of a participant under 18 years of age, hereby acknowledge and agree to the following terms regarding my or my child's use of, participation in activities at, or visit to the First Tee - Virginia Blue Ridge Youth Development and Golf Performance Facility ("The Clubhouse”):
I acknowledge that participation in golf activities, practice, events, facility use, or other activities at the First Tee - Virginia Blue Ridge Youth Development and Golf Performance Facility involves inherent risks, including but not limited to the risk of injury, illness, or disease (including exposure to COVID-19 or other communicable diseases). I voluntarily assume all risks associated with the use of the Facility or participation in any activities.
I agree to release, indemnify, and hold harmless the Blue Ridge Youth Golf Foundation, Inc., First Tee - Virginia Blue Ridge, First Tee National Headquarters Office, and their respective officers, directors, employees, agents, LPGA and PGA Professionals, participating agencies, and volunteers from any and all claims, demands, or causes of action of any nature arising from or related to my (or my child's) participation in activities or use of the Facility, including transportation, whether caused by negligence or otherwise.
In the event of an emergency, I authorize First Tee - Virginia Blue Ridge staff, representatives, or volunteers to secure necessary medical treatment for myself or my child, understanding that I am responsible for any associated costs.
I consent to First Tee - Virginia Blue Ridge and First Tee National Headquarters Office communicating information regarding participation, programs, events, and opportunities through electronic means, including email, text messages, and other online platforms.
I acknowledge that all equipment provided by the Facility, including but not limited to golf clubs, practice tools, and technology, is the property of First Tee - Virginia Blue Ridge.
I agree to handle all equipment with care and use it only for its intended purpose.
I understand that any abuse, misuse, or negligent handling of equipment, including intentional damage, will result in my responsibility for the cost of repair or replacement.
The Facility reserves the right to deny access to equipment if misuse is observed or suspected.
I further agree to return all borrowed equipment promptly at the end of each activity in the same condition in which it was received, normal wear and tear excepted.
Youth participants ages 12 and under must be accompanied by a parent, legal guardian, or other responsible adult while using the Facility.
Teen participants ages 13 through 18 may use the Facility without direct adult supervision at the discretion of their parent or legal guardian, provided the parent or legal guardian signs the participant in and has executed all required waivers and consent forms.
Parents or legal guardians remain responsible for the conduct of their child or teen and for ensuring compliance with all Facility rules and policies while the youth participant is on the premises.
First Tee - Virginia Blue Ridge reserves the right to deny facility access or participation to any minor participant who does not meet the supervision and sign-in requirements outlined above.
I grant permission to First Tee - Virginia Blue Ridge and its representatives to capture photographs, video, or other media of myself or my child during activities at the Facility or related events. I understand that these images or recordings may be used in promotional, educational, or program-related materials, including but not limited to brochures, social media, websites, newsletters, and other communications supporting the mission of First Tee. I waive any right to inspect or approve the final product and understand that I will not receive compensation for the use of such media.
ACKNOWLEDGMENT AND AGREEMENT
By signing below, I acknowledge that I have read and fully understand this waiver and release of liability. I affirm that I am authorized to sign on behalf of myself or my child and agree to the terms and conditions outlined above.
